Silicone has many medical applications. It is a theoretically inert substance that is found in nature.  Due to its hydrophobic (water-repelling) properties, silicone does not readily integrate into human tissue and thus does not allow tissue ingrowth into an implantable device.  These properties make silicone a theoretically ideal filler or medical device. In practice, however, silicone injections and silicone fillers have many long term complications.  Unlike more common facial fillers such as Juvederm, Restylane, and Radiesse, Silicone (Silicone, Silikon) is a permanent tissue filler and thus does not dissolve with time.  Thus, if a patient is unhappy with the result, or if there are undesired soft-tissue reactions such as swelling, redness, and lumpiness, it is difficult to remove from the body.


In our plastic surgery practice in Los Angeles, we receive patients from around the world who have undergone silicone injection in various parts of the body.  The lips, hips, thighs, and buttocks are common areas of injection. Patients with silicone in their bodies may desire removal of the silicone because of unwanted swelling, redness, infection, inflammation, pain, discomfort, and ulceration.  These are very difficult problems to solve and a regimented protocol is necessary to acheive acceptable results and maintain aesthetic contour. 


Treatment of Silicone in the Lips:


Silicone is often injected into the upper and lower lip areas.  Unlike temporary lip augmentation with Juvederm, Restylane, Collagen, or Hyaluronic acid, silicone does not dissolve with time.  To the contrary, silicone is a permanent filler that not only stays in the body, but often develops into bigger inflammatory nodules that are known as granulomas.  Some patients will also develop non-nodular swelling, itching, burning, and pain that is directly attributed to the silicone itself.


The treatment of lip silicone or lip nodules is difficult.  Treatment begins with the injection of corticosteroids to decrease the inflammatory effects of the silicone itself.   These injections should be performed by a physician who is familiar with the treatment of silicone in the lips, as complications can occur.  In practice, silicone that is minimally-responsive to corticosteroids can be surgically removed using a variety of techniques.   These procedures are usually performed in an outpatient surgical suite under anesthesia.

Injectable silicone has become an increasingly popular method of volume enhancement of the face and body.  Patients have sought treatment in the United States and abroad, often with drastic complications including silicone nodularity, silicone granulomas, silicone migration, silicone-induced inflammation, silicone-induced subacute infection, silicone extrusion, and silicone visibility with gradual atrophy and aging of the surrounding soft tissue.


Silicone is a theoretically inert substance in the human body and this quality makes it appealing to medical device manufacturers who exploit the hydrophobic properties of silicone polymers in medical devices.  Most of these silicone medical devices contain a more viscous form of silicone and are inserted into the body in a surgically sterile fashion.   In terms of silicone injections, the same does not hold true.  Patients who have undergone silicone injections are often unaware of the source of the injectable silicone, the purity of the injection, the technique of injection, or the qualifications of their clinician.  This uncertainty often places patients at risk of short and long term silicone complications as the contaminants contained within the injectable silicone may cause subacute infection or inflammation.

Q: Is there a laser that can be used to remove silicone from the lips or body? 

A: Patients with silicone complications often ask whether there is an advanced laser treatment to vaporize silicone from the body if it is causing lumps and granulomas.  As yet, there are no specific lasers that can work on the silicone without affecting the surrounding tissue.  In some rare cases of lip silicone injections, Dr. Raffy Karamanoukian can use a laser guided CO2 laser to remove a segment of the lip granuloma during surgery.  If you are having problems with your silicone injections on the lip or body, Dr. Karamanoukian can discuss the surgical and non-surgical options with you during your plastic surgery consultation. 

Q: Is silicone permanent? 

A: Silicone injections will likely stay in your body for a lifetime.  There is no absorption of silicone and silicone injections can only be removed by surgical resection.

Q: Are there any celebrities with silicone in their lips? 

A: There are several very famous celebrities who have admitted to having silicone in their lips. These can often be removed with meticulous surgical resection.  Lip silicone be removed using an outpatient technique that has been refined and perfected by Dr. Karamanoukian.

Q: Can I have Juvederm or Restylane injected after my silicone is removed?

A: Most patients will benefit from a safer and temporary augmentation of the lips after silicone injection removal.

Q: Can Bio-Alcamid, PMMA, Artefill, and silicone all be removed from the lips? 

A: Bio-Alcamid, PMMA, Artefill, and silicone are all permanent fillers that can cause a severe reaction in the soft tissue of the lips and face. Often, the tissue reaction can lead to changes that are physically deforming. Treatment can be performed by Dr. Karamanoukian for all different types of permanent soft tissue fillers. The first step in determining your candidacy for surgery is to obtain an appointment for consultation. During your visit, Dr. Karamanoukian will examine your lips and determine whether you are a candidate for surgical excision or non-surgical management.


There are many different types of facial fillers on the market today.  It is always important to go to a recognized expert when you are having volume enhancement of the face and body with injectable fillers.  The following discussion provides some insight into facial fillers as well as protocols to treat complications from facial fillers.  Remember, facial fillers have a very good safety profile and are an excellent source of volume enhancement.  There use, however, should always conform to recognized standards of patient care to optimize safety.


Fortunately, the commercial availability of temporary facial fillers such as Juvederm and Restylane have decreased the incidence of long term complications associated with facial fillers.  However, there are still many patients who have undergone or who still undergo facial augmentation with more permanent facial fillers such as Silicone (Silicon, Silikon), Artefill, or Artecoll.  The availability of temporary facial fillers and their popularity are no consolation for patients who suffer the longterm complications of permanent facial fillers, such as nodularity, lumps, bumps, swelling, discoloration, Tyndall effect, and poor aesthetic result.

Collagen Facial Filler: Collagen is a recognized gold-standard in facial volume enhancement but about 3% of patients developed an allergy to the bovine (cow-derived) collagen, necessitating pretreatment allergy testing.  Bovine collagen was the original soft tissue filler available in the United States under the tradenames Zyderm and Zyplast.  Results are excellent with collagen injection, however, pretesting is important to avoid the complication of subdermal antigens.


Fat Grafting: Fat grafting or fat augmentation is merely in its infancy as advancing techniques have propelled the excellent long term technique to fame.  Fat grafting is theoretically possible on any part of the body and provides stem cells as well as active adipocytes for transplantation.  Fat is first suctioned using liposuction and purified under centrifuge before injection into the face or body.  The Brazilian Butt lift incorporates the fat grafting techniques to naturally enhance buttock fullness.  As with any other filler, complications on the donor site and recipient site are possible if precautions are not met.


Restylane: Restylane has a very good track record for safety.  It is made up of biosynthetic hyaluronic acid derived from bacteria.  It is safe to use on the lips and other areas with thin skin such as the eyelids.  Complications are not permanent with Restylane. If you are having problems with your Restylane injection, you may be a candidate for a hyaluronidase injection which is an enzyme that reverses the Restylane injection. 


Can injectable silicone be removed from my lips?  The answer to this question is difficult.  Usually, it is not the silicone itself that is causing the deformity of the lips, but rather, the granulomatous or inflammatory reaction that surrounds the silicone. Treatments include surgical removal of the silicone, anti-inflammatory modulators, TAC injection, and conservative treatments, but surgery is usually limited to particular patients who have silicone isolated in specific anatomic compartments of the wet mucosa.  In more severe cases, removal of the silicone may cause further destruction of the normal lip contour and function.  Dr. Karamanoukian has extensive experience in silicone removal from the lips. 


How do I know if I am a candidate for surgical removal of silicone nodules? Silicone is ubiquitous as a permanent filler in many parts of the world.  Dr. Karamanoukian sees patients who come to Los Angeles from all over the world in search of a treatment for silicone injection complications.    Dr. Karamanoukian also receives many emails requesting a consult via video log or sending of digital pictures.  In most cases, it is difficult to ascertain the true nodular deformity through pictures but our office will make a good faith attempt to evaluate your individual case.  However, Dr. Karamanoukian recommends a complete consultation and evaluation if the decision to perform surgery is to be discussed.
